Appointments

It is important that you are prepared for your appointment and  that you bring the necessary items with you when you arrive for your appointment.

If you are a first time patient please bring:

Medical records – If you are visiting us because your physician has referred you to us please bring your medical records, x-rays, test results or other relevant materials that will be helpful to us in evaluating your condition.

Arrive at least 15 minutes early in order to give you time to fill out any new patient paperwork or if you are a returning patient time to review your existing information to assure that it is still accurate. Allowing this extra time before your scheduled appointment will also allow us to stay on schedule and see you on time as well.

Other important information to bring with you:

Personal Medical History (This includes past history on your physical condition, any medications that you may be taking and specific information on your current medical condition)

Insurance Information (Bring your current insurance card)

Workman’s Compensation Information (If a work related accident or condition)

Payment - Insurance co-payment is required at time of visit. If you do not have insurance then payment in full will be required at the time of your appointment or after treatment has been received. Arrangements must be made in advance for any charges, which you are unable to pay above $200. For hip, leg or ankle exam, please bring a pair of shorts to wear.
